In case you are stuck with an issue such as your account getting debited but no cash disbursed at ATM, you can then download and fill in the declaration form with complete details of your transaction made, account number, your 16 digits card number, amount debited from the concerned account, exact location of the ATM and name of the bank. It is better if you include your transaction decline slip given by the ATM (if available). And you will receive a response from the bank within 7 days of the information provided to the bank.
